Payment is due at the time of service unless other arrangements have been made. I will provide you with a superbill to submit to your insurance for payment. It is your responsibility to call your insurance and establish if you have a deductible and/or coinsurance that needs to be met, as well as finding out what percentage they will pay for out of network care.
